Since it'd be hard to integrate new findings with old ones while keeping credits straight I made this page here for stuff I've found on my own. Its not a whole lot but maybe it'll grow over time?

Errors:
Vol.1
• pg.216: Morganna is misspelled "Morgana" twice on this page.
Vol.2
• pg.119: Lios is mislocalized here as "Ryos".
• According to the internets, Hokuto's players real life name is Haruka Mizuhara. However in AI buster Vol.2, the title for her story is called "Haruka Muzuhara". Dunno where that extra "u" came from!
Lost in translation:
Vol.1
• Watari's newbie assistant is incorrectly referred as male. Apparently in the japanese version his assistants gender was never specified.
Mutation
Errors:
• For some odd reason "Goblin Gloves A" are instead written as "Goblingloves A". This is corrected for Outbreak/Quarantine.
• In the CC Corp mai "From the Grunty Ranch" a little bit of text towards the end goes beyond the mail windows border. This error isn't present in Outbreak/Quarantine.
• During the scene in which Lios is introduced, one of BlackRose's lines has a typo, spelling "someone" as "somone".
• Monster #213 in Ryu Book IV is called "Dalaigon Anecdote". However when you encounter it in the flesh, its name is displayed as "Dalaigon Anecdot".
• Prior to the Cubia fight, Kite has a dialogue box which displays "!!O". The "O" is likely unintentional.
• In the news story "Tokyo Mega Float Operation Begins" a line towards the end of it says "will began operations". This is corrected to "will begin operations" in Outbreak & Quarantine.
• The "RE: Jogging" mail from Moonstone has a typo "50 back strech" which is probably supposed to be stretch/streches.
Outbreak
Errors:
• Monster #213 in Ryu Book IV is called "Dalaigon Anecdote". However when you encounter it in the flesh, its name is displayed as "Dalaigon Anec".

Vol.1
• The translation stays consistent with the errors in .hack//Infections translation regarding Alph. Her name is once again spelled "Alf" and she is called a he. Being consistent is good, but that doesn't mean its not wrong!
• pg.217: BlackRose unleashes the skill "Ray rack" which I'm pretty sure is supposed to be "Rairaku".
• pg.227: "ignoramus" is used instead of "pigheaded" in reference to Lios. However later on in the following volumes its they use "pigheaded".
Vol.2
• pg.117: Chimney talks about himself and Nova as if they're both Blademasters, but Nova's a Heavy Blade.
Vol.3
• pg.27: "The church was as I remembered it. It was the second time I'd been there, and this time I dared to come alone instead of with Kite." This is wrong because on pg.176 of Vol.2 its stated she'd returned to the church, and alone. Making it her third visit there and second time by herself.
• pg.70: BlackRose thinks "Could it be the fifth Phase?" This is an error because at that point they'd only defeated three Phases!
• pg.97: "I envied her the job, given my feelings about ghosts." That "the" shouldn't be there!
• pg.225: Once again Nova is refered to as a Blademaster!
Vol.4
• pg.46: "Don't worry about it so much, Asaoka. What will be, will be." The one saying that was Asaoka who wasn't talking to herself. She was talking to Akira.
• pg.212: "When we reached floor B5, the cat that had appeared at the dungeon entrance once again came into view." This is incorrect because in the game the cat reappears on B7.
Legend of the Twilight
Errors:
Vol.2
• pg.27: Hunter's Bane is referred to as a spell which raises a partys accuracy. This is wrong, it's a scroll which decreases monsters physcial accuracy.
• pg.79: "Yeah. Her name's Zelfie." no, it's Zefie!
• back cover: Rena is misspelled "Reina".
Vol.3
• pg.27: Theres an instance of Zefie being spelled "Zephie" here.
• pg.49: Balmung refers to Net Slum's master as "him" but isn't Helba a female?
• pg.95: "...something unknown, something dangerous, lose in The World". I believe "lose" is almost certainly supposed to be "loose" and they just typo'd here.
The End of The World (Terminal Disc)
Edits:
• The edit which changed Skeith's wand from a cross to a Q shape in SIGN and IMOQ shows up here as well.
Errors:
• During Interim Report 1, Alph's name is finally properly localized! But... because of this its now an inconsistent with IMOQ and Another Births which used "Alf".
• During Interim Report 3, it's stated Cubia appears for a 3rd battle with Kite which is wrong. Minutes later this
is corrected when Jun says "After a long and diffcult second battle..."
• During Interim Report 11, Gord is mislocalized as "Godo" both in spelling and pronunciation. Also Sagisaka isn't quite right nor is B Set (should be Sakisaka, Gord, and Bset).
Cell
Errors:
Vol.1
• pg.47: "Perhaps it was in inner ear problem." That "in" is probably meant to be an "an".
• pg.53: There's a small typo where Gaspard is instead called "Gaspar".
• pg.57: On this page Grein is called a Blade Brandier. However he is actually an Edge Punisher.
• pg.73: "She simple drifted, lazily, endlessly." The "simple" was most likely meant to be "simply".
• pg.85: The white Harvest Cleric has a "girlish voice" yet the text that follows repeatedly refers to the person as male. This character is clearly Kaho (confirmed in Vol.2 pg.85 strangely enough!) who's a girl in real life and whose character also is one.
• pg.111: Theres a strange lil typo in here where a capital "i" is placed before Midori's name with no space between them making it read as "IMidori".
• pg.148: And it happens again but as "IThe"!
• pg.150: "While player-killing is permitted, doing it to this extend results in objections being raised,". "extend" seems like a typo which was probably supposed to be "extent".
Vol.2
• pg.21: "she titled her head curiously, as she examined her own body." "titled" doesn't sound right so its likely a typo of "tilted"!
G.U. Vol.1: Rebirth
Errors:
• In the official forum thread "Working people fall in!" the player Saltaholic's name is mispelled as "Soltaholic". His guild name Saltmania is mispelled as "Sultmania".
• Under Ryu Book IV you can view "Weapons", "Armor", "Accessories", and "Items in Storage". The last one being an error which isn't corrected until Vol.3 where it is changed to "Items".
• There are several errors within Hokuto's first email ("Lena", "Emma Willant's", "her"). Nameiwse, Lena should be Rena while Emma Willant should be Emma Wielant. The "her" part I can only guess its Watari that Hokuto is talking about and so it should be "him" instead.
• There's a forum thread titled "Job of Multi-Weapon" which should probably instead be "Job of Adept Rogue".
• Trying to use a Fairy's Orb in certain places will display the message: "This item cannot be used him." Why him? I dunno! But it should be "here".
• "Fault" is used as the Chapter name for both 14 & 16. This is an error as Chapter 16 should've been named "Rogue".
• In various forum threads people talk about the Medic Union members saying "nyan" but in the game itself they say "meow".
G.U. Vol.2: Reminisce
Errors:
• There's a forum thread titled "Job of Multi-Weapon" which should probably instead be "Job of Adept Rogue".
• Trying to use a Fairy's Orb in certain places will display the message: "This item cannot be used him." Why him? I dunno! But it should be "here".
• In various forum threads people talk about the Medic Union members saying "nyan" but in the game itself they say "meow".
• Pao: I'll hold onto that Lucky Ball and Animal Radar since you won't be needing it.
• Kubo: I'll hold onto the Chim Chim Radar for now.
The problem with these two is that Lucky Ball & Chim Chim Radar were the names of the items found in Vol.1. In this game they were called Lucky Sphere & Chim Examiner.
• Captain: Congratulations on your victory at the Demon Palace. To go to Hy Brasail, choose "Set Sail." Except its the Holy Palace you won in this volume!
• Captain: "Did you have a good time at the party? To go to Hy Brasail, choose "Set Sail". This is said even when you're already at Hy Brasail!!
• The people running the Guild Shops in Dol Dona for Moon Tree, Kestrel, Medic Union, Trifle and Empireo have no dialogue. This is obviously an error since in Vol.3 they do dialogue when spoken to there.

• After getting a Job Extension one of the notifications that popup will say you've received a "Scythe Slice". Problem is, theres no weapon with that name! The one ya end up with in your inventory is "Scythe Shouxiao"!
• The Vol.3 preview video doesn't have any subtitles. This is strange since the one for Vol.2 did.
• During one of the Abyss events an NPC says "I am...Habuzi...Havshi the "Decomposed" So is it Habuzi or Havshi?
• The skill "Rip Mumyn" is now spelled "Rip Mumin". Why? "Mumyn Lei" was a skill in IMOQ which induced Sleep, and this skill here cures Sleep so why the sudden name deviation?
• In the greeting for the knights guild shops the clerk says "show" when obviously that should be "shop".
Lost in translation:
• In the Japanese PS2 version of this game, "The World" log in screen displayed the version number 2.98. However in the localized game, it remained 2.26 which Rebirth had used.
G.U. Vol.3: Redemption
Errors:

• The NPC Aira has some missing dialogue in the early chapters of the game:
Chapters 1-5: "I'm the oldest of the PKing triplets, Aira. I have two younger sisters."
Chapters 6-37: "I'm the oldest of the PKing triplets, Aira. I have two younger sisters. The middle one is a Steam Gunner named Hiira, and the youngest is a Twin Blade named Kiira."
This is odd because her dialogue had remained unchanged through both Rebirth and Reminisce and is still technically unchanged here.
• The description for the added effect Saturate Self is "Gradually increase SP (recovers more that SP Regen 2)". "More that". Not "than"? Making it more apparent this is a typo is that the HP version of this effect uses "More than".
• The NPCs Encephalon and IOTA both have some dialogue text which run off the screen, which makes for some incomplete sentences.
• In the Apkallu thread Cuteness Up, a broadsword called "Centipede" is discussed. Vol.3 wallpaper 038 also calls it "Centipede". However there is no sword by that name! That item is clearly the "Broad Legged"!
• Pao: "This magical item is called a Lucky Ball. I invented it myself." and "If you can fill up the Lucky Ball with data, I'll be quite happy." and "I'll hold onto that Lucky Ball and Animal Radar since you won't be needing it."
The problem here is that Lucky Ball was the name of the item found in Vol.1. In this game it was called Luck Sphere.
• Dr. Kubo: "I'll hold onto the Chim Chim Radar for now."
The problem here is that Chim Chim Radar was the name of the item found in Vol.1. In this game it was called Chim Data Radar.
• Late in the game after exiting out of a cutscene in Raven you'll be reminded to go to the area "Sigma Fleeing Dismayed Emotion". The odd thing here is that the area name is displayed with a purple font color which is strange since area names are always in a golden color!
• In an even later late game cutscene there's a subtitle error in which the text "...Man, what was that all about?" is used twice despite not matching the spoken dialogue.
• In a post-game quest a thing called the "Gear of Malice" is brought up. In dialogue form it has that name but in system message windows its called "Gear of Evil" which is pretty weird!
• In a couple of emails the "Dusty Miller" item is instead called a "White Chrysanthemum" for some odd reason.
• The skill "Rip Mumyn" is now spelled "Rip Mumin". Why? "Mumyn Lei" was a skill in IMOQ which induced Sleep, and this skill here cures Sleep so why the sudden name deviation? Making this even stranger than the Vol.2 iteration of this error, is that in the skill description its called "Rip Mumyn". So you'll see, on the same screen, both "Rip Mumin" and "Rip Mumyn"!
• Idle dialogue from Shino in Breg Epona includes: "I wonder what Sakisaka or B-set are doing?" in which I'm pretty sure the "or" is supposed to be an "and".
• Following that she also says "Godo's... Doing his best too, I bet." Godo? That' Gord! At least this mispelling is consistent with the Terminal Discs mispelling!
• In the greeting for the knights guild shops the clerk says "show" when obviously that should be "shop".
G.U. Last Recode
Edits:
• Vol.3: A monster once named "Captain Hook" is now named "Captain Crook".
• Vol.3: Chaotic PKer "Lady Bomber Jahad" has been renamed "Explosive Gia".
• Vol.3: One of Cubia's attacks was once named "Jihad" in both IMOQ and the original release of G.U. Vol.3 (and Another Birth iirc). This has now been changed to "Mjöllnir" because??? Thanks for breaking continuity?
Errors:
• Vol.1: Rip Mumyn is now Rip Mumin. In the original Vol.1 both the skill name and its description used Rip Mumyn. In Vol.2 it became Rip Mumin. In Vol.3 the skill name remained Rip Mumin, but in its description it went back to being called Rip Mumyn.
• Vol.1/2: the first Alchemy Factory tutorial message uses [Cronos Spike] + [Cronos Spike] = [Cronos Spike +1]. In the original Spin Gai Gu was used... probably because there is no weapon in the game called Cronos Spike! This madeup/mislocalized weapon first appeared in Vol.2 where the NPC IGA mentions receiving one.
• Vol.1: Mumyn Lei has become Munim Lei which is just... what?
• Vol.1: In a very early cutscene video "8 Months Later" shows up in the video. However in the subtitle it says "8 Month Later".
• Vol.1/2: There's a translation error which says you can Alchemize equipment which are up to 20 levels apart (a feature introduced in the original Vol.3). However if you try to do this, you'll be told you can't because Alchemized equipment only works with a 10 level difference between them. (not present in Vol.2 HD)
• Vol.1/2/3: The Movie Player pages are now 01 to 05. Originally they had used 00 to 04. Also odd because the BGM/Wallpaper numbering still begins with 00.
• Vol.2: At the very start of the game, parts of the NPC Tiphereth's dialogue no longer displays. Text inside parenthesis is what got eaten:
-> We're inside of "The World" right now (, aren't we?)
Ah, I don't mean that we're playing "The World" (, I mean that we actually exist inside of it now.)
I'm sorry, it's hard for me to say... Do you get what I'm trying to say?

• Vol.2: When alchemizing, or exiting then re-entering the Alchemize menu may cause the space where Haseo's name should be, to instead be blank, the name of the party member you had last selected in the alchemy menu before exiting it, or some very strange characters/symbols.
• Vol.2: The level requirement for Spin Indigo is incorrect. It says 61 while actually being 62.
• Vol.2/3: Ryu Book VII now lists the monster "Hell Charm" as a L61 - 70 monster, whereas in the originals it was listed at L71 - 80.
• Vol.2/3: In the greeting for the knights guild shops the clerk says "show" when obviously that should be "shop".
• Vol.3: The text after collecting the last Symbol Fragment used to be "The Symbol Fragments have started to shine..." This has been changed to "Symbol Fragments are starting to shine." This slight text change found in the original Vol.3 is not present in the HD version.
• Vol.3: Malphas is now listed as being a L111 - 120 monster whereas before it was L121 - 130.
Lost in Translation:
• Vol.1: The first Chapter title "First Day" has been changed to "First Cry". The second Chapter title has been changed from "deja vu" to "Deja Vu".
• The subtitle for the Blade Brandiers Sword Flash skill says "Shadow Flash". 11 year old mistranslation, or recent mistranslation?
• Vol.3: After the main stories over you'll get an email from someone called Ono and within it theres a difference between the old and new version of the game which also creates an error in the new version because there is no "thanksgiving" to select!
-> PS2: If you'll accept, please select "The Sweepstakers" in a Limit Battle in the Sage Palace.
-> PS4: If you'll aceept, please select "thanksgiving" in a Limit Battle in the Sage Palace.
Online Jack
Errors:
• During Episode 8 "The World" login screen is shown and the version number 2.26 is on it. This is a continuity error since it should match Vol.3's 3.58 version number!
G.U. Returner
Errors:
• The arrangement of the desktop items is different from the G.U. games. In the games the order goes "The World", Mail Station, Crimson VS, News Capture, Community Forum, Movie Player, Accessories, and Data Manager. Here however it appears to be "The World", News Capture, Data Manager, Accessories, Movie Player, Mail Station, Community Forum, and Crimson VS.
(Note: This was never localized but I felt this inconsistency was worth noting.)
G.U. Vol.4: Reconnection
Lost in Translation:
• "Its way different from the old days when you were champion." Some flubbed localization consistency here! In Japanese its arena champion but in english it was always arena emperor. Its technically not an error an yet it is one in some way!
G.U. Trilogy
Errors:
• "Aina" is mis(re?)localized as "Ina" in this movies subtitles.
G.U. (Novels)
Errors:
Vol.1
• pg.188: There's a typo which has "Kun" instead of "Kuhn".
Lost in translation:
• Throughout all of Vol.1 Haseo's real life name is spelled "Ryou". But in each of the following ones its "Ryo".
Project X Zone
Errors:
• One of BlackRose's attacks "Gun Drive" is actually supposed to be "Gan Drive".
• Some typo/grammar errors are easily spotted in some dialogue when paired up with Devilotte.
Devilotte: "The World, a realm in cyberspace... Someday it will be mine as well!"
BlackRose: "I wouldn't try it if I would you. That would make us enemies."
Kite: "A quest to defeat the princess... That's sounds pretty innovative, actually."
• Dual Swords are mispelled as "Duel Swords".
Lost in translation:
• The key words for the cathedral are said to be "Hidden Forbidden Sanctum" instead of the "Hidden Forbidden Holy Ground" we've all come to know and love. Allegedly "sanctum" is a proper translation and the original .hack games localization are in the wrong, but they still should've went with "holy ground" to remain consistent with the western releases.
• Skeith's wand retains its cross shape rather than being editted into a Q shape as it had been in previous localized .hack media.
